Accident Report FormFingerprinting Information
Per California state law, HTLL volunteers must be fingerprinted through the U.S. Department of Justice. For privacy reasons related to viewing the results of the scan, volunteers must be fingerprinted for each organization separately, including HTLL. State law says that fingerprints given for another organization (e.g., work, non-profit, etc) are not visible to HTLL and do not satisfy the requirement to be fingerprinted for HTLL.
How to get fingerprintedStep 1: Download and print the HTLL Livescan information form. This form has HTLL DOJ registration number on it. Fill out the other areas of the form.
Download Livescan FormStep 2: Take the Livescan form to any Livescan facility of your choosing to complete the fingerprinting. That's it.

Getting clearedThe results will be sent to the league safety officer only. The safety officer will only communicate a 'clear' or 'not clear' reading to the league. No details of the result will be divulged to anyone else in the organization.
ReimbursementLivescan is not free and each facility sets their own price for the service. Unfortunately, it is not possible at this time for HTLL to pay to have its volunteers fingerprinted. However, HTLL will reimburse volunteers up to $20 if they wish. Present the original receipt to a board member who will pass it along to the Treasurer for payment. HTLL thanks you for your understanding.
